Falls are one of the leading causes of injury, loss of independence, and hospital admission in older adults. For many people, a single fall — or even the fear of falling — can have a profound impact on confidence, mobility, and quality of life. The good news is that falls are not an inevitable part of ageing. With the right assessment and targeted physiotherapy, the risk of falling can be significantly reduced, and confidence with movement can be restored.

Falls prevention and elderly rehabilitation are at the core of Physio Assist's work. Our lead clinician has extensive NHS experience in this field, working in community rapid response teams, intermediate care units, hospital at home services, and specialist falls prevention services across the UK.

Our Approach

A comprehensive falls risk assessment

We begin with a comprehensive falls risk assessment in the patient's own home — assessing not just strength and balance, but the environment they live in. This includes:

  • Standardised balance and mobility tests (Timed Up and Go, Berg Balance Scale, and others).
  • Muscle strength assessment, with a focus on lower limb function.
  • Gait analysis — walking pattern, speed, and confidence.
  • Home hazard review — identifying trip hazards and recommending practical modifications.
  • Review of footwear, walking aids, and relevant medications.

A personalised rehabilitation programme

From this, we build a personalised rehabilitation programme including:

  • Progressive balance and strengthening exercises — carefully designed to challenge safely and build confidence.
  • Functional movement practice — getting up from chairs, managing stairs, navigating the home safely.
  • Walking and gait re-training — improving pattern, stride, and outdoor confidence.
  • Advice for carers and family members to support progress between sessions.

Care Home Visits

We provide regular physiotherapy visits to care homes, working with residents on an individual or group basis:

  • Individual falls risk assessments and personalised exercise programmes.
  • Group exercise sessions — gentle, chair-based exercise to maintain strength, balance, and mobility.
  • Staff guidance — advising care home teams on supporting residents' mobility safely.
  • Post-fall review — assessment and rehabilitation planning following an incident.
